Itchy painful bumps on the lower legs

A 55-year-old female patient is on your surgery list and has sent in the following photograph via the AccuRx system ahead of the appointment. She reports that these lumps have developed on both of her lower legs in a symmetrical pattern – they are firm to the touch and when they started, they were small itchy red bumps. They are intensely itchy causing her to scratch the skin and cause some injured areas that she is concerned are now getting infected. She is worried as some of the lesions that have settled down are leaving some scarring to the skin.
